Responsibilities
- Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and toileting.
- Provide companionship and engage clients in meaningful conversations and recreational activities.
- Prepare nutritious meals and assist with meal planning according to dietary restrictions.
- Administer medication reminders and assist with light housekeeping tasks to maintain a safe environment.
- Monitor client health and report any changes in condition or behavior to the Case Manager immediately.
- Accompany clients to medical appointments, errands, and social outings.
Qualifications
- High school diploma or GED is preferred.
- CPR and First Aid certification is highly desirable.
- Valid Driver's License and reliable transportation are required.
- Pass a comprehensive background check and drug screen.
- Strong communication skills and a patient, empathetic demeanor.
- Previous experience in home care, nursing assistance, or personal care is a plus.
