Responsibilities
- Provide remote patient monitoring and health assessments via telehealth technology
- Assist with medication management and treatment adherence support
- Coordinate care plans with registered nurses and interdisciplinary teams
- Document patient interactions accurately in EMR systems
- Respond to patient inquiries and provide emotional support
- Participate in virtual team huddles and continuing education
Qualifications
- Active Ohio CNA certification in good standing
- Minimum 1 year clinical experience (remote preferred)
- Proficiency with telehealth platforms and EMR systems
- Strong communication skills for virtual patient interactions
- Ability to work independently while collaborating remotely
- CPR/BLS certification
- Reliable high-speed internet and dedicated home workspace
