Responsibilities
- Provide direct patient care and assistance with activities of daily living (ADLs) such as bathing, dressing, and grooming.
- Monitor vital signs, including blood pressure, temperature, pulse, and respiration, and report any abnormalities to the nursing staff immediately.
- Assist patients with mobility, transfers, and ambulation using proper body mechanics and transfer equipment.
- Maintain a clean, safe, and sanitary environment for patients, ensuring all infection control protocols are followed.
- Document patient care activities accurately and in a timely manner within electronic health records (EHR).
- Communicate effectively with patients, families, and the healthcare team to ensure continuity of care.
- Administer medications and treatments as directed by the Registered Nurse (RN) or Licensed Practical Nurse (LPN).
Qualifications
- Current and active Certified Nursing Assistant (CNA) license in the state of Pennsylvania.
- Current Basic Life Support (BLS) and Cardiopulmonary Resuscitation (CPR) certification.
- Strong interpersonal and communication skills with a focus on patient empathy.
- Ability to work flexible schedules, including nights, weekends, and holidays, as required.
- Reliable transportation for home health or facility assignments.
- High school diploma or GED equivalent preferred.
- Experience in a hospital, long-term care, or home health setting is a plus.
