Responsibilities
- Provide daily personal care assistance including bathing, dressing, and grooming
- Monitor vital signs and report changes in patient condition to nursing staff
- Assist with mobility transfers and safe ambulation using proper techniques
- Document patient care activities and observations accurately in electronic health records
- Support patients with nutritional needs and meal preparation assistance
- Participate in interdisciplinary care planning meetings
- Maintain a clean, safe, and organized patient environment
Qualifications
- Active Wisconsin CNA certification in good standing
- High school diploma or equivalent required
- Minimum 1 year recent acute care or long-term care experience preferred
- Current CPR/BLS certification
- Ability to lift up to 50 pounds and stand/walk for extended periods
- Strong communication and interpersonal skills
- Attention to detail and ability to follow care plans precisely
- Flexibility to work varied shifts including weekends and holidays
