Responsibilities
- Conduct virtual patient assessments and health monitoring via telehealth platforms
- Coordinate care plans between patients, families, and clinical teams
- Provide remote medication reminders and adherence support
- Document patient interactions accurately in electronic health records (EHR)
- Facilitate secure video consultations with RNs and physicians
- Respond to patient inquiries via secure messaging systems
- Collaborate with on-site staff to ensure continuity of care
Qualifications
- Active Pennsylvania CNA certification required
- Minimum 1 year clinical experience in hospital or long-term care
- Proficiency with telehealth software (e.g., Zoom for Healthcare, Epic MyChart)
- Strong written communication skills for patient documentation
- Ability to maintain HIPAA compliance in remote work environment
