Responsibilities
- Monitor patient health status and vital signs via secure virtual platforms and report changes to the supervising Registered Nurse.
- Assist patients with daily living activities, personal care, and hygiene as required in a remote support capacity.
- Maintain accurate and up-to-date patient records using Electronic Health Records (EHR) systems.
- Communicate effectively and empathetically with patients, families, and interdisciplinary healthcare teams.
- Ensure strict compliance with all state and federal healthcare regulations, HIPAA guidelines, and facility policies.
- Perform basic health assessments and document observations accurately in real-time.
- Collaborate with case managers to ensure continuity of care.
Qualifications
- Active and valid Certified Nursing Assistant (CNA) license in Pennsylvania.
- Current Basic Life Support (BLS) and Cardiopulmonary Resuscitation (CPR) certifications.
- Proficiency in Microsoft Office Suite and Electronic Health Records (EHR) software.
- Strong computer skills with the ability to navigate remote work platforms independently.
- Excellent verbal and written communication skills.
- High school diploma or GED required; Associate degree in Nursing preferred.
