Responsibilities
- Provide high-quality patient care via telehealth monitoring and virtual support
- Assist with daily living activities (bathing, dressing, mobility) during in-home visits
- Accurately document patient health records using our digital EHR system
- Coordinate care plans with RNs and interdisciplinary teams remotely
- Administer medications and treatments under clinical supervision
- Monitor vital signs and report changes to healthcare providers
- Support patient education and wellness programs virtually
Qualifications
- Active NC CNA certification in good standing
- Minimum 1 year recent clinical experience
- Proficient with telehealth platforms and EHR systems
- Strong communication and documentation skills
- Ability to travel for 2-3 in-home visits per week
- CPR/BLS certification required
- Pass background check and drug screening
- Valid NC driver's license and reliable transportation
