Responsibilities
- Conduct virtual health assessments via telehealth platform
- Coordinate patient care plans with RNs and physicians
- Provide medication reminders and symptom monitoring
- Deliver empathetic patient education and support
- Maintain accurate electronic health records (EHR)
- Collaborate with multidisciplinary care teams
- Respond to patient inquiries within SLA timeframe
- Participate in quality improvement initiatives
Qualifications
- Active CNA certification in good standing
- 1+ years clinical experience (remote preferred)
- Proficient with telehealth platforms (e.g., Teladoc, Amwell)
- Strong communication and documentation skills
- Ability to pass background check and drug screen
- Experience with EHR systems (Epic or Cerner)
- Compassionate patient-centered approach
- High-speed internet and quiet workspace
