Responsibilities
- Patient Monitoring: Observe and record patient health status, reporting changes or concerns to the nursing team immediately.
- Assisted Living Support: Assist patients with activities of daily living (ADLs) including bathing, dressing, grooming, and mobility assistance.
- Care Coordination: Communicate effectively with patients, families, and healthcare providers to ensure holistic care plans are met.
- Medication Assistance: Support patients with medication reminders and administration as directed by licensed nurses.
- Hygiene & Safety: Maintain a clean, safe, and sanitary environment for patients in accordance with health regulations.
- Documentation: Maintain accurate and up-to-date clinical records and logs.
Qualifications
- Certification: Active and valid CNA license in the State of California.
- CPR/BLS: Current Basic Life Support (BLS) certification for Healthcare Providers.
- Experience: Minimum of 1 year of experience in a clinical or home health setting preferred.
- Communication: Excellent verbal and written communication skills.
- Technology: Proficient in using electronic health record (EHR) systems and basic computer applications.
- Compassion: A genuine passion for helping others and a strong patient-centric approach.
