Responsibilities
- Provide compassionate personal care assistance to patients including bathing, dressing, and mobility support
- Monitor vital signs and document patient observations accurately in our digital health system
- Assist with medication administration under RN supervision
- Coordinate telehealth appointments and remote patient monitoring tasks
- Maintain clean, safe environments for patients and staff
- Collaborate with interdisciplinary care teams via virtual huddles
- Support patient education and emotional wellbeing initiatives
Qualifications
- Active Pennsylvania CNA certification required
- Minimum 1 year clinical experience in home care or facility setting
- CPR/First Aid certification (or willingness to obtain)
- Proficiency with telehealth platforms and digital documentation tools
- Flexible availability including evenings, weekends, and holidays
- Strong communication skills with ability to work autonomously
- Valid Pennsylvania driver's license and reliable transportation
