Responsibilities
- Provide compassionate personal care assistance to patients including bathing, dressing, and mobility support
- Monitor vital signs and document patient observations accurately in our electronic health records system
- Assist with medication administration under nurse supervision
- Collaborate with interdisciplinary teams to develop and implement patient care plans
- Maintain a safe, clean, and organized patient environment
- Support patients with nutritional needs and meal assistance
- Participate in shift handovers and team huddles via telehealth platforms
Qualifications
- Active Tennessee CNA certification in good standing
- Minimum 1 year of recent clinical experience in long-term care or hospital setting
- Current CPR/BLS certification
- Proficient in electronic health record systems with remote access capabilities
- Strong communication skills with ability to document clearly via remote portals
- Ability to work flexible shifts including evenings, weekends, and holidays
- Valid Tennessee driver's license and reliable transportation
- Pass background check and drug screening
