Responsibilities
- Provide compassionate assistance with daily living activities including bathing, dressing, and feeding
- Accurately monitor and document patient vital signs and health status changes
- Assist with patient mobility transfers using proper techniques to ensure safety
- Collaborate with nursing teams to develop and implement individualized care plans
- Maintain a clean, organized, and infection-controlled patient environment
- Support emotional well-being through active listening and patient advocacy
- Participate in interdisciplinary care conferences and quality improvement initiatives
Qualifications
- Active New York State CNA certification in good standing
- Minimum 1 year of recent acute care or long-term experience preferred
- Current BLS/CPR certification (American Heart Association)
- Excellent communication and interpersonal skills
- Ability to stand/walk for extended periods and lift up to 50 lbs
- Strong attention to detail and documentation abilities
- Commitment to maintaining HIPAA compliance and patient confidentiality
- Positive attitude and passion for geriatric or specialized care
