Responsibilities
- Provide compassionate personal care (bathing, dressing, grooming) to patients in home/community settings
- Monitor vital signs and report changes in patient condition to clinical staff
- Assist with mobility, transfers, and ambulation using proper techniques
- Administer medications as prescribed and document accurately
- Support activities of daily living (ADLs) while maintaining patient dignity
- Collaborate with interdisciplinary care teams to ensure continuity of care
- Maintain clean, safe environments for patients and staff
Qualifications
- Active Ohio CNA certification in good standing
- Minimum 6 months recent CNA experience preferred
- Valid CPR/BLS certification (or willing to obtain)
- Ability to work flexible shifts including nights/weekends
- Strong communication and interpersonal skills
- Physical ability to lift/patients up to 50 lbs
- Pass background check and drug screening
- Reliable transportation for home visits
