- Remote Documentation: Accurately record patient information, treatment plans, and progress notes via our secure telehealth portal.
- Communication: Act as the primary point of contact between patients, families, and medical staff to ensure seamless care coordination.
- Emergency Response: Remain calm and efficient during medical emergencies, following all safety protocols and emergency response guidelines.
- Health Education: Educate patients and their families on disease management, medication adherence, and healthy lifestyle choices.

Qualifications
- Certification: Current and active Certified Nursing Assistant (CNA) license in the State of Texas.
- Certifications: Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications required.
- Experience: Minimum of 1-2 years of experience in a clinical or home health setting preferred.
- Skills: Proficiency in electronic health records (EHR) and telehealth platforms.
