Responsibilities
- Provide compassionate, patient-centered care including bathing, dressing, and mobility assistance
- Monitor vital signs and document patient conditions via our secure remote portal
- Assist with medication administration under RN supervision
- Collaborate with healthcare teams through virtual care coordination meetings
- Maintain accurate electronic health records (EHR) from remote locations
- Support patients with activities of daily living (ADLs) in home or facility settings
- Respond to emergency situations following established protocols
Qualifications
- Active Arizona CNA certification in good standing
- Minimum 1 year experience in home healthcare or assisted living
- Proficient with telehealth platforms and EHR systems
- Valid CPR certification (or willingness to obtain)
- Ability to work independently with strong time-management skills
- Compassionate approach with excellent communication abilities
- Flexibility to adapt to dynamic night shift schedules
- Reliable transportation for in-home patient visits
