Responsibilities
- Monitor and document patient vital signs via telehealth platforms
- Assist with daily living activities (ADLs) through virtual care coordination
- Provide emotional support and health education to patients remotely
- Collaborate with RNs and physicians on patient care plans
- Maintain accurate electronic health records (EHRs)
- Respond to patient inquiries via secure messaging systems
- Participate in virtual case conferences and care reviews
Qualifications
- Active California CNA certification
- Basic computer literacy and internet access
- Strong communication skills (verbal/written)
- Compassion and patience for elderly/patient care
- Ability to work independently in a remote setting
- Pass background check and drug screening
- High school diploma or equivalent required
- Experience with telehealth platforms preferred
