Responsibilities
- Deliver high-quality virtual patient assessments and care coordination via telehealth platform
- Provide health coaching and education for chronic disease management (diabetes, hypertension, etc.)
- Collaborate with multidisciplinary teams to develop personalized care plans
- Document patient encounters accurately in EHR system following HIPAA protocols
- Monitor patient progress and escalate critical cases to clinical supervisors
- Participate in virtual health fairs and community wellness initiatives
- Maintain strict compliance with Illinois nursing board regulations
Qualifications
- Active Illinois RN license (or compact license with Illinois residency)
- New graduates from accredited nursing programs strongly encouraged to apply
- Basic computer proficiency and willingness to learn telehealth platforms
- Strong communication skills and empathy for diverse patient populations
- Critical thinking ability for remote clinical decision-making
- Flexibility to adapt to evolving telehealth protocols
- Valid BLS certification (or willingness to obtain within 30 days)
