Responsibilities
- Provide compassionate virtual patient assessments via telehealth platform
- Develop and implement individualized care plans for assigned patients
- Collaborate with interdisciplinary teams to coordinate remote care delivery
- Monitor and interpret patient data from remote monitoring devices
- Document clinical encounters accurately in electronic health records
- Educate patients on health management and medication protocols
- Respond to patient inquiries and triage emergent situations
Qualifications
- Active California RN license (or compact state eligibility)
- New graduates with < 1 year experience encouraged to apply
- BSN degree or enrollment in accredited BSN program
- BLS certification required; ACLS preferred
- Proficiency with telehealth technology and EHR systems
- Exceptional communication and critical thinking skills
- Ability to work independently in remote environment
