Responsibilities
- Provide remote patient assessments, consultations, and care coordination via telehealth platforms
- Collaborate with interdisciplinary teams to develop and implement patient care plans
- Monitor patient progress and document clinical outcomes in electronic health records
- Educate patients on health conditions, medications, and preventive care
- Respond to patient inquiries and triage urgent situations appropriately
- Participate in continuous quality improvement initiatives
Qualifications
- Active RN license in Tennessee (or compact state)
- Associate or Bachelor's degree in Nursing (ADN/BSN)

- Strong communication and critical thinking skills

- Ability to work independently in a remote setting
