Responsibilities
- Provide telehealth nursing consultations and patient education
- Coordinate care plans with multidisciplinary teams
- Document patient interactions accurately in EHR systems
- Monitor patient health metrics remotely
- Respond to patient inquiries via phone, email, and secure messaging
- Collaborate with case managers for chronic disease management
- Participate in virtual team huddles and training sessions
Qualifications
- Associate's or Bachelor's degree in Nursing (ASN/BSN)
- Active Colorado RN license (or compact state eligibility)
- New graduates or nurses with <1 year experience encouraged
- Basic computer proficiency with EHR systems
- Strong communication and critical thinking skills
- Ability to work independently in a remote setting
- CPR/BLS certification
