Responsibilities
- Deliver virtual patient education on care plans, medications, and health management via telehealth platform
- Monitor patient progress through digital tracking tools and report critical changes to clinical teams
- Provide empathetic remote support to patients and families during sensitive health transitions
- Maintain accurate, HIPAA-compliant electronic health records (EHR) documentation
- Collaborate with RNs and providers to coordinate seamless virtual care delivery
- Participate in virtual training sessions to enhance clinical skills and telehealth proficiency
- Respond to patient inquiries via secure messaging within agreed SLA timeframes
Qualifications
- Active Washington State CNA certification
- High school diploma or equivalent (college coursework preferred)
- Reliable home office setup with high-speed internet and quiet workspace
- Proficiency with telehealth software, EHR systems, and digital communication tools
- Exceptional verbal/written communication skills and emotional intelligence
