Responsibilities
- Deliver remote patient assessments via telehealth platforms
- Coordinate care plans with multidisciplinary teams
- Monitor patient vitals and medication compliance remotely
- Document electronic health records accurately
- Provide patient education via virtual consultations
- Participate in on-site clinical rotations (2 days/week)
- Collaborate with case managers for discharge planning
- Adhere to HIPAA compliance protocols
Qualifications
- Associate's or Bachelor's in Nursing (BSN preferred)
- Active Illinois RN license or eligibility
- NCLEX-RN pass (within 6 months of hire)
- CPR/BLS certification
- Basic proficiency with EHR systems
- Strong communication skills
- Ability to work independently
- Flexibility for occasional on-site shifts
