Responsibilities
- Conduct virtual health assessments via telehealth platform
- Monitor patient vitals and medication adherence remotely
- Coordinate care plans with interdisciplinary teams
- Provide emotional support to patients and families
- Document care progress in electronic health records
- Respond to patient inquiries within 2-hour SLA
- Collaborate with on-site care providers for complex cases
Qualifications
- Active Texas CNA certification
- 1+ years clinical experience preferred
- Strong computer literacy with digital health tools
- Excellent communication skills via video conferencing
- Ability to work independently with minimal supervision
- Valid Texas driver's license (for occasional on-site visits)
- CPR/BLS certification
- Pass background check and drug screening
