Responsibilities
- Provide comprehensive telehealth assessments and patient consultations via secure video platform
- Develop and implement personalized care plans for diverse patient populations
- Coordinate care transitions between in-person and virtual settings
- Document clinical encounters accurately in electronic health records (EHR)
- Collaborate with multidisciplinary teams to ensure continuity of care
- Monitor and respond to patient inquiries through secure messaging systems
- Participate in quality improvement initiatives and compliance monitoring
Qualifications
- Active NYS RN license with at least 2 years of clinical experience
- Bachelor of Science in Nursing (BSN) preferred
- Experience with telehealth platforms or virtual care delivery
- Strong proficiency in EHR systems (Epic, Cerner, etc.)
- Excellent communication skills with ability to build rapport remotely
- Certification in telehealth or digital health preferred
- Ability to work independently with minimal supervision
- Valid DEA license for controlled substance prescribing
