Responsibilities
- Provide daily personal care assistance including bathing, grooming, and dressing support to ensure client dignity and hygiene.
- Assist clients with mobility, transfers, and walking to promote safety and independence.
- Monitor and accurately document vital signs, mood changes, and overall health status in electronic health records.
- Prepare nutritious meals and assist with feeding according to specific dietary restrictions.
- Administer medication reminders and light housekeeping tasks as directed by the supervising nurse.
- Offer emotional support and companionship to clients to enhance their quality of life.
- Collaborate with the care team to develop and adjust individualized care plans.
Qualifications
- High School Diploma or GED required.
- Current state certification as a Home Health Aide (HHA) or Personal Care Assistant (PCA) is mandatory.
- Valid Driver’s License and reliable transportation for client travel.
- Ability to pass a comprehensive background check and drug screening.
- Strong interpersonal and communication skills with a patient, empathetic demeanor.
- Physical stamina to lift, bend, and stand for extended periods.
- CPR and First Aid certification preferred.
