Responsibilities
- Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, and grooming.
- Monitor patient health and report any changes in condition to the Registered Nurse (RN) or Case Manager.
- Administer medication and perform basic wound care as trained and directed.
- Provide companionship and emotional support to patients and their families.
- Assist with mobility, transfers, and ambulation using proper body mechanics.
- Prepare nutritious meals and assist with light housekeeping and laundry.
- Maintain a clean, safe, and organized environment for patients.
Qualifications
- High School Diploma or GED is required.
- Current Home Health Aide (HHA), Certified Nursing Assistant (CNA), or Personal Care Assistant (PCA) license.
- Valid driver's license and reliable transportation are required for home visits.
- CPR and First Aid certification.
- Strong interpersonal and communication skills.
- Ability to pass a background check and drug screening.
- Compassionate, patient, and dependable work ethic.
