Responsibilities
- Assist clients with activities of daily living (ADLs) including bathing, dressing, grooming, and toileting.
- Monitor and accurately record vital signs, weight, and changes in the patient's physical or mental condition.
- Provide companionship and emotional support to ensure patients feel comfortable and engaged.
- Assist with light housekeeping, meal preparation, and feeding as needed.
- Administer medication and follow care plans strictly as directed by RNs or LPNs.
- Transport clients to medical appointments, social activities, or errands.
- Document all care activities and communicate concerns to the nursing team immediately.
Qualifications
- Current Massachusetts Home Health Aide (HHA) or Certified Nursing Assistant (CNA) license.
- High school diploma or GED equivalent required.
- Valid driver's license and reliable personal transportation.
- Strong communication and interpersonal skills with a patient-centered approach.
- Ability to work flexible hours, including nights, weekends, and holidays.
- Physical stamina and ability to lift and assist patients with mobility.
