Responsibilities
- Conduct virtual patient assessments and vital monitoring via telehealth platforms
- Assist physicians with clinical documentation and electronic health record management
- Coordinate patient care plans and medication reconciliation remotely
- Facilitate secure patient communication through our HIPAA-compliant portal
- Prepare and maintain medical supplies for periodic in-person clinic visits
- Support patient education and chronic disease management programs
- Collaborate with interdisciplinary teams for comprehensive care coordination
Qualifications
- Certified Medical Assistant (CMA) or Registered Medical Assistant (RMA) required
- Minimum 2 years clinical experience in ambulatory care setting
- Proficiency with telehealth platforms (e.g., Teladoc, Amwell)
- Strong knowledge of EHR systems (Epic, Cerner preferred)
- Excellent communication skills for virtual patient interactions
- Ability to travel to San Jose clinic bi-weekly for hands-on support
- Current BLS/CPR certification
- California Medical Assistant certification required
