Responsibilities
- Provide compassionate assistance with activities of daily living including bathing, dressing, and mobility support
- Monitor vital signs and document client observations accurately in medical records
- Assist with medication administration reminders and meal preparation
- Transport clients to medical appointments and social activities using agency vehicles
- Implement individualized care plans under supervision of RN case managers
- Maintain a safe, clean environment and infection control protocols
- Communicate effectively with families and healthcare team members
Qualifications
- Current California CNA certification in good standing
- Minimum 6 months of recent geriatric care experience
- Valid driver's license with clean driving record
- Ability to lift 50+ pounds and stand for extended periods
- Clear TB test and health screening within past year
- Proficient in English with strong documentation skills
- Compassionate demeanor and reliable transportation
- Flexibility to work weekends and evenings as needed
