Responsibilities
- Conduct virtual patient assessments and develop personalized care plans
- Monitor chronic conditions via telehealth platforms and adjust treatment protocols
- Collaborate with multidisciplinary teams using EHR systems for seamless coordination
- Educate patients on medication adherence and preventive care measures
- Document clinical encounters with precision and maintain HIPAA compliance
- Respond to urgent patient inquiries during assigned shifts
- Participate in continuous quality improvement initiatives
Qualifications
- Active Oregon RN license with at least 2 years of clinical experience
- Telehealth certification or willingness to obtain within 90 days
- Proficiency in EHR platforms (Epic, Cerner, or similar)
- Strong critical thinking and decision-making skills
- Excellent verbal and written communication abilities
- Ability to work independently while collaborating effectively
- Flexibility to work evenings, weekends, and holidays as needed
- BLS certification required
