Responsibilities
- Provide exceptional remote and in-person patient assessments, care planning, and interventions
- Collaborate with multidisciplinary teams via telehealth platforms for coordinated care delivery
- Administer medications and treatments with precision while maintaining accurate documentation
- Monitor patient progress and adjust care plans based on clinical outcomes
- Educate patients and families on health management and preventive care
- Adhere to HIPAA compliance and maintain strict confidentiality standards
- Participate in quality improvement initiatives and evidence-based practice
Qualifications
- Active Oregon RN license in good standing
- Minimum 2 years of clinical nursing experience
- BLS certification; ACLS preferred
- Experience with telehealth platforms and EHR systems
- Strong critical thinking and problem-solving abilities
- Excellent communication skills for virtual and in-person interactions
- Ability to adapt to flexible shift schedules including evenings/weekends
