Responsibilities
- Deliver high-quality virtual patient assessments and care planning via telehealth platform
- Monitor and manage chronic conditions through remote consultations and digital health tools
- Coordinate care transitions between in-person providers and virtual services
- Document encounters accurately in EHR systems with strict HIPAA compliance
- Provide patient education on medication adherence and lifestyle modifications
- Collaborate with multidisciplinary teams to optimize care pathways
- Participate in quality improvement initiatives and clinical protocol development
Qualifications
- Active Oregon RN license with minimum 2 years clinical experience
- Telehealth or virtual care experience strongly preferred
- Proficiency in EHR systems and telehealth technology
- Strong assessment skills for remote patient evaluation
- Certification in Basic Life Support (BLS)
- Excellent communication skills for virtual patient interactions
- Ability to work independently with minimal supervision
- Experience in chronic disease management preferred
