Responsibilities
- Deliver patient assessments and care coordination via telehealth and in-person visits
- Monitor chronic conditions and provide health education remotely
- Collaborate with interdisciplinary teams using Epic EHR systems
- Manage medication reconciliation and treatment protocols
- Document patient encounters with precision and HIPAA compliance
- Participate in virtual case reviews and quality improvement initiatives
- Maintain flexible availability across assigned shift blocks
Qualifications
- Active Oregon RN license (compact license preferred)
- 2+ years clinical experience in primary care or telehealth
- Proficiency with telehealth platforms (e.g., Teladoc, Amwell)
- BLS certification; ACLS preferred
- Strong critical thinking and crisis management skills
- Excellent written/verbal communication abilities
- Ability to adapt to changing shift requirements
- Experience with EHR documentation systems
