Responsibilities
- Deliver high-quality patient care via telehealth and in-clinic settings
- Assess patient health conditions and develop personalized care plans
- Collaborate with multidisciplinary teams to coordinate patient services
- Utilize electronic health records (EHR) systems for documentation
- Provide health education to patients and families remotely
- Monitor patient progress and adjust care plans as needed
- Comply with HIPAA regulations and healthcare protocols
Qualifications
- Active RN license in Wisconsin (WI)
- BSN degree or equivalent experience
- 2+ years of clinical nursing experience
- Telehealth certification or willingness to obtain
- Strong assessment and critical thinking skills
- Experience with EHR systems (Epic preferred)
- Excellent communication and technology proficiency
- Ability to work independently in remote settings
