Responsibilities
- Provide virtual patient assessments and care coordination via telehealth platform
- Collaborate with multidisciplinary teams to develop patient care plans
- Monitor and interpret patient data using remote monitoring systems
- Deliver health education and medication management support virtually
- Maintain accurate electronic health records (EHR) documentation
- Respond to patient inquiries and triage emergent situations
Qualifications
- Active Georgia RN license required
- Minimum 2 years clinical nursing experience
- BLS certification; ACLS preferred
- Experience with telehealth platforms (e.g., Teladoc, Amwell)
- Strong computer literacy and EHR proficiency
- Exceptional communication and critical thinking skills
- Ability to work independently with minimal supervision
