Responsibilities
- Deliver high-quality patient care via telehealth and in-person visits
- Monitor patient vitals, symptoms, and medication adherence remotely
- Collaborate with multidisciplinary teams for integrated care plans
- Document patient interactions accurately in EHR systems
- Provide health education to patients and caregivers

Qualifications
- Active Washington State RN license
- Minimum 2 years clinical nursing experience
- Telehealth certification or willingness to obtain
- Strong proficiency with EHR platforms (e.g., Epic, Cerner)
- Excellent communication and critical thinking skills
- Ability to work independently with minimal supervision
- Flexibility to cover varied shifts (evenings, weekends, holidays)
- BLS/CPR certification
