Responsibilities
- Deliver high-quality patient assessments and care plans via telehealth platforms
- Monitor patient vitals and medication adherence remotely using digital tools
- Provide clinical guidance and health education to patients and caregivers
- Collaborate with interdisciplinary teams to coordinate patient care
- Document patient encounters accurately in electronic health records
- Respond to urgent patient inquiries during scheduled shifts

Qualifications
- Active RN license in Minnesota
- Minimum 2 years of clinical nursing experience
- Proficiency with telehealth platforms and EHR systems
- Strong critical thinking and problem-solving abilities
- Excellent communication skills for virtual patient interactions
- Ability to work independently and manage time effectively
- Certification in BLS (Basic Life Support)
- Experience in chronic disease management preferred
