Responsibilities
- Deliver virtual patient assessments and triage services via telehealth platform
- Collaborate with multidisciplinary teams to develop personalized care plans
- Document patient encounters accurately in EHR systems
- Provide clinical education and health coaching to patients remotely
- Monitor chronic conditions and coordinate follow-up care
- Respond to patient inquiries within SLA timeframes

Qualifications
- Active WI RN license (compact license accepted)
- 2+ years clinical experience in acute care or telehealth
- Proficient with EHR systems and telehealth technology
- BLS certification required; ACLS preferred
- Exceptional communication and critical thinking skills
- Ability to work independently with minimal supervision
- Experience with chronic disease management preferred
- Strong problem-solving abilities in virtual settings
