Responsibilities
- Assess patient symptoms via telehealth and phone triage to determine urgency and appropriate care pathways
- Provide accurate clinical guidance to patients while maintaining empathy and cultural sensitivity
- Collaborate with physicians and specialists to coordinate seamless care transitions
- Maintain detailed electronic health records (EHR) with precise documentation
- Educate patients on home care protocols and preventative health measures
- Adhere to HIPAA compliance and institutional protocols for all interactions
- Participate in continuous quality improvement initiatives
Qualifications
- Active RN license in Texas (TX) with minimum 2 years acute care experience
- BLS certification and ACLS preferred
- Proven expertise in telephone triage protocols and clinical judgment
- Exceptional communication skills with ability to de-escalate sensitive situations
- Proficiency in EHR systems (e.g., Epic, Cerner)
- Ability to work independently with strong time management skills
- Spanish bilingual certification highly desirable
- Experience with telehealth platforms preferred
