Responsibilities
- Conduct rapid patient assessments via phone and telehealth to determine urgency of care needs
- Provide evidence-based medical advice and triage recommendations per established protocols
- Collaborate with physicians and care teams to coordinate appropriate patient pathways
- Maintain accurate electronic health records and documentation
- Delicate patient education on self-care and next steps
- Monitor and report trends in patient inquiries to improve service quality
Qualifications
- Active RN license in California with minimum 2 years clinical experience
- BLS certification; ACLS preferred
- Proven triage experience in emergency, urgent care, or telehealth settings
- Exceptional critical thinking and decision-making abilities
- Strong communication skills with diverse populations
- Proficient in Epic EHR systems
- Ability to work 20-30 hours/week with flexible shifts
