Responsibilities
- Conduct thorough patient assessments via phone, email, and secure messaging to evaluate symptoms and determine appropriate levels of care.
- Utilize clinical protocols to triage incoming requests, directing patients to urgent care, primary care, or emergency services as necessary.
- Maintain accurate and up-to-date documentation of all patient interactions and clinical findings within the Electronic Health Record (EHR) system.
- Facilitate timely referrals to specialists and coordinate follow-up appointments to ensure continuity of care.
- Provide clear patient education and health counseling to promote wellness and self-management of chronic conditions.
- Collaborate closely with physicians, nurse practitioners, and the wider care team to resolve patient issues efficiently.
Qualifications
- Current, unencumbered Registered Nurse (RN) license in the State of California required.
- Minimum of 2 years of nursing experience, with prior triage experience strongly preferred.
- Certifications: BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) must be active.
- Strong verbal and written communication skills with a focus on patient-centered care.
- Proficiency in using Electronic Health Records (EHR) and telehealth platforms.
- Ability to work a flexible part-time schedule, including evenings and weekends.
