Responsibilities
- Provide high-quality virtual patient assessments and telehealth consultations
- Develop and implement individualized care plans via remote monitoring systems
- Collaborate with multidisciplinary teams through digital health platforms
- Coordinate care transitions between in-person and virtual settings
- Maintain accurate electronic health records using Epic systems
- Participate in continuous quality improvement initiatives
- Provide patient education through secure video conferencing
Qualifications
- Active RN license in Massachusetts (compact license preferred)
- Minimum 2 years clinical nursing experience
- BLS and ACLS certifications required
- Proficiency with telehealth platforms (Teladoc, Amwell)
- Experience with Epic EHR systems
- Strong critical thinking and decision-making abilities
- Excellent written and verbal communication skills
