Responsibilities
- Provide comprehensive remote patient assessments via telehealth platforms and virtual monitoring systems
- Develop and execute individualized care plans for diverse patient populations
- Coordinate interdisciplinary care teams including physicians, specialists, and case managers
- Document patient encounters accurately in electronic health records (EHR) systems
- Provide patient education and health coaching through virtual channels
- Respond to urgent patient inquiries and triage emergent situations remotely
- Participate in quality improvement initiatives for remote care delivery
- Maintain compliance with HIPAA and telehealth regulations
Qualifications
- Active RN license in Oregon (or compact state)
- Minimum 2 years acute care nursing experience
- Bachelor of Science in Nursing (BSN) preferred
- Telehealth certification or equivalent experience
- Proficiency with EHR systems and virtual care platforms
- Strong critical thinking and decision-making abilities
- Excellent communication skills for virtual interactions
- Ability to work independently with minimal supervision
