Responsibilities
- Conduct comprehensive patient assessments and triage via telehealth and remote monitoring platforms.
- Administer medication and treatment plans as prescribed by physicians and healthcare providers.
- Document patient interactions, vital signs, and treatment outcomes accurately in the EMR system.
- Coordinate patient care with interdisciplinary teams, including physicians, therapists, and social workers.
- Educate patients and families on disease prevention, health maintenance, and medication management.
- Respond to patient inquiries and escalations with urgency and professionalism.
Qualifications
- Current, unencumbered Registered Nurse (RN) license in the District of Columbia (or compact state eligible).
- Associate Degree in Nursing (ADN) required; Bachelor of Science in Nursing (BSN) preferred.
- Certifications: BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) required.
- Minimum of 2 years of clinical nursing experience, preferably in a hospital or acute care setting.
- Proficient in electronic health record (EHR) systems and telehealth technologies.
- Strong communication, critical thinking, and time management skills.
