Responsibilities
- Provide high-quality patient care via telehealth platforms for diverse medical cases
- Conduct virtual assessments, develop care plans, and monitor patient progress
- Collaborate with interdisciplinary teams to coordinate patient services
- Document clinical encounters accurately in our electronic health record system
- Educate patients on health management and medication adherence
- Respond to patient inquiries and triage urgent situations appropriately
- Participate in continuous quality improvement initiatives
Qualifications
- Active Registered Nurse license in Ohio (or compact state)
- Minimum 2 years of clinical nursing experience
- Experience with telehealth or remote patient care preferred
- Strong assessment and critical thinking skills
- Excellent communication and documentation abilities
- Proficiency with electronic health record systems
- BLS certification required
- Ability to work independently with minimal supervision
