Responsibilities
- Deliver virtual patient assessments and consultations via HIPAA-compliant telehealth platforms
- Monitor and interpret remote patient monitoring data for critical interventions
- Collaborate with multidisciplinary teams to develop personalized care plans
- Provide patient education on chronic disease management and medication adherence
- Maintain accurate electronic health records in Epic systems
- Respond to patient inquiries and triage emergent situations
- Participate in quality improvement initiatives and clinical protocols
Qualifications
- Active RN license in Washington DC (compact license accepted)
- Minimum 2 years of clinical nursing experience
- BLS and ACLS certifications required
- Telehealth or remote care experience preferred
- Strong proficiency with EHR systems (Epic preferred)
- Exceptional communication and critical thinking skills
- Ability to work independently with minimal supervision
