Responsibilities
- Conduct thorough patient assessments and triage via secure telehealth platforms.
- Collaborate with physicians and care teams to develop and implement patient care plans.
- Document patient interactions, vital signs, and treatment plans accurately in the Electronic Health Record (EHR).
- Provide patient education and emotional support to individuals and families regarding health conditions.
- Monitor patient status and escalate urgent medical issues to the appropriate clinical staff immediately.
- Participate in quality improvement initiatives and patient safety protocols.
Qualifications
- Graduation from an accredited Nursing Program (Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN)).
- Current, unencumbered Registered Nurse (RN) license issued by the State of California.
- CPR/BLS certification (Advanced Cardiovascular Life Support (ACLS) preferred).
- Strong computer skills and proficiency with electronic health record systems.
- Excellent communication and interpersonal skills with a focus on empathy.
- Ability to work independently and manage time effectively in a remote setting.
