We are looking for a detail-oriented professional to manage patient triage, assessments, and follow-up care in a remote setting.
Responsibilities
- Conduct comprehensive patient health assessments via secure video conferencing and phone calls.
- Provide remote triage and clinical guidance for acute and chronic conditions.
- Document all patient interactions accurately and promptly in the electronic health record (EHR).
- Collaborate with physicians and care teams to develop and adjust treatment plans.
- Educate patients on disease prevention, medication management, and healthy lifestyle choices.
- Ensure strict adherence to HIPAA regulations and clinical protocols.
Qualifications
- Active and unencumbered Registered Nurse (RN) license in North Carolina (or compact state).
- Minimum of 2 years of clinical nursing experience (Telehealth, ICU, or Med-Surg preferred).
- Current Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications.
- Proficiency in using EMR/EHR systems and telehealth platforms.
- Strong communication skills with the ability to build rapport remotely.
- Reliable high-speed internet and a dedicated workspace.
