Responsibilities
- Provide high-quality patient care through telehealth monitoring and virtual consultations
- Collaborate with multidisciplinary teams to develop personalized care plans
- Document patient interactions accurately in electronic health records (EHR)
- Conduct remote health assessments and medication management
- Participate in continuous quality improvement initiatives
- Maintain HIPAA compliance in all virtual communications
- Provide patient education and health coaching remotely
Qualifications
- Active RN license in good standing (Colorado required)
- Minimum 2 years of clinical nursing experience
- BLS certification; ACLS preferred
- Experience with telehealth platforms and EHR systems
- Strong critical thinking and communication skills
- Ability to work independently with minimal supervision
- Flexibility to work some evening/weekend shifts
- Bachelor's in Nursing (BSN) preferred
