Responsibilities
- Provide high-quality patient assessments and develop individualized care plans using telehealth technology and remote monitoring tools
- Administer medications and treatments accurately while maintaining detailed electronic health records
- Collaborate with multidisciplinary teams to coordinate patient care across virtual and in-person settings
- Educate patients and families on health management and disease prevention strategies

Qualifications
- Active RN license in the state of Arizona
- Minimum 2 years of clinical nursing experience in acute care or telehealth
